This guide is for 香港六合彩 employees who are the primary parent in a couple who are adopting a child. Please refer to the policy for full details of adoption leave entitlement and eligibility.听
Before you start
There are two things you need to do to apply for adoption leave:听
- notify your manager, preferably in writing, and听
- fill out the .听
Both need to be done at the earliest opportunity.听
HR will write to you within four听weeks of receiving your form. 听
To begin your application for adoption leave you will need to know:听
- your child's adoption date听
- the date you would like to begin your leave听
- when you expect to return to work听
- 1. Fill out the online adoption leave form

- Fill out the .听
- Please ensure that you have a copy of the original adoption certificate听ready to attach to the form. If you are still waiting for the certificate, please send a copy of this to hr.services@ucl.ac.uk as soon as you receive it.
- Once you have completed your form, it will be automatically sent to your line manager to sign off.
- 2. Form goes to your Line Manager/PI

- Your line manager will receive the form automatically via email once you have completed your听section of the online form.
- Your line manager听will then need to sign the form after they听have been able to听provide听details of how they propose to cover your duties.
- If you are grant funded, the PI section is designed to act as a visual prompt for your PI to notify your funder to ensure that your Grant funding is not impacted by you going on leave.
- Once they have signed the form, it will automatically听be sent to HR Services to process your application.
- 3. Form goes to your line manager

- Your line manager will receive the form automatically via email once you have completed your听section of the online form.
- Your line manager听will then need to sign the form after they听have been able to听provide听details of how they propose to cover your duties.
- Your line manager will also need to attach proof that they have conducted a Pregnancy Risk Assessment, by attaching the PDF downloaded from听riskNET to the form when prompted to do so.
- If you are grant funded, the PI section is designed to act as a visual prompt for your PI to notify your funder to ensure that your Grant funding is not impacted by you going on leave.
- Once they have signed the form, it will automatically听be sent to HR Services to process your application.
